The Mind altar teleport is a type of Magic tablet which allows you to teleport directly outside the Mind altar.
It is purchasable by exchanging 32 Runecrafting guild tokens earned through the minigame The Great Orb Project with Wizard Elriss. However, to obtain these tablets, an indirect requirement of level 50 Runecrafting is required to enter the Runecrafting Guild and be able to play The Great Orb Project to purchase them.
It is useful for reaching the Dwarven Mine or Goblin Village quickly, as well as hunting Green Dragons in the Wilderness, though a Chaos altar teleport would be better for that purpose.
