The Anti-poison totem is an item bought from the rewards trader in Daemonheim for 44,000 Dungeoneering tokens. To equip it, a player must have 60 Defence, 60 Dungeoneering and 70 Herblore. It is worn in the shield slot.
It doesn't work like antipoison, as it will not cure poison. However, a player wearing this will be immune to being poisoned. The totem's defensive bonuses are similar to those of a Rune sq shield, but the melee defence bonuses are a bit lower and it provides no Damage soaking. It has no negative bonuses, a +5 magic defence, and more Summoning defence.
The immunity to poison can be very useful against poisonous Slayer creatures (such as Cave crawlers, Kalphites, and Jungle strykewyrms), as it allows more room for food and drops. It can also save a player from death when doing hard and elite Clue scrolls, to ward off the poison of Saradomin wizards and Guthix wizards. The totem can be used during Barbarian Assault for immunity against Penance Healers, and during Pyramid Plunder for immunity to snakes and scarab swarms. It may also be used to avoid being poisoned while training on Monkey Guards or skeletal apes on Ape Atoll. It may also be helpful while fighting the Dagannoth kings to provide protection from the spinolyp. One of the most useful features of the totem is providing immunity against K'ril Tsutsaroth's poison (starting at 160), as it reduces the need for antipoison potions and thus allowing space for food (among other things) in your inventory. The only poison this item does not protect against is that of the Monstrous cave crawler.
This item will appear at your gravestone if you die, so do not be afraid of losing it in the event of death.

- Although this item has significant defensive bonuses, the monks will allow players to take it onto Entrana.
