Guthan the Infested is the third oldest of the seven Barrows brothers. He attacks with melee, and his set effect allows him to heal himself at a 1:1 ratio, so if he deals 200 life points and the effect occurs, he would recover 200 life points. His effect is visible; three ghostly entities surround Guthan's opponents then circle around them in an upwards motion.

Some players "tank mage" Guthan. This means that they cast spells such as slayer dart while wearing their melee armour, such as rune, granite, etc. Even though wearing metal while maging reduces your mage bonus significantly, Guthan's magic defence is so poor that you will be hitting almost as accurate as with Mystic or other robes. This strategy is used so he won't be able to hit you often, thus, reducing the chances of healing himself. Alternatively some people use the protect from melee prayer which avoids the need to wear armour while spell casting and blocks all of Guthan's damage and most of his healing.

Black Salamander is recommended as it is not affected by the player's magic attack bonus too much. However, slayer dart may be used as a substitute, unless the player has a high magic level, in which a Polypore staff may be used.

Trivia

• Guthan, along with all the other barrows brothers, received a graphical update on 4 January 2012 to make him look more like a player wearing his items rather than the old graphics style which displayed an unclear, purely pink coloured apparition. Along with this update, his combat level was increased from 115 to 147.
• When Guthan's face disappears, most of the face will disappear, but after a tick, a small black shape will appear, and disappear in the next tick.
