The Dragon ceremonial cape was a rare reward from the Squeal of Fortune. It is one of five parts to the whole set. This set along with the Queen's Guard outfit could only be won "throughout the summer months" according to Jagex.
One of the unique features of this item is that, when the entire set is complete, it can be re-coloured when you wear all 5 items by right clicking. This ability is also shared with the Dragon Ceremonial clothes. "Re-colouring is a first for cosmetic items of this nature" according to Jagex.

Trivia
- There appears to be a glitch when wearing this cape. It often seems to render the person invisible to other players. All that is visible is the white dot in the mini-map. Even mousing the cursor over where the person is does not register anything. There are two ways to fight an invisible player; keep auto retaliate on or go to the same square where the invisible player is, then right click it. Though players may use this to their advantage during PvP activities, there have been reports of players getting banned for abusing this glitch.
- Another glitch is when you chose to wield it while you have another cape on, it will 'copy' the last cape you wore, and show up on the player as the other cape when you wear it.
