A blood necklace is a somewhat rare drop from an Edimmu, the level 90 Slayer monster in Daemonheim. Given the low spawn rate of Edimmu and the low necklace drop rate, it is not uncommon for a player to not see one for dozens of millions of experience. Its special ability activates every 15 seconds, dealing 40 (or less depending on Combat or Constitution level, doing maximum damage at 91 Constitution.) damage to all enemies directly adjacent to the player but only if the monster is in combat with a player (does not have to be the player with the necklace). Any damage dealt this way will heal the player by the same amount. It currently has the second highest strength bonus for an amulet only exceeded by Saradomin's whisper, but cannot be taken outside of Daemonheim.

It is one of two items currently available within Daemonheim that is worn in the necklace slot, the other being the Seeker charm. In practice, its special is an effective way to partially reduce food consumption and to deal slightly more damage to piles of opponents.

Though mages and rangers may appreciate its stat bonuses, the special requires close proximity to potential targets and so it is somewhat counterintuitive to use it when attacking from a distance. For this reason the necklace is a better bind for meleers than rangers or mages.

Owners of the blood necklace should do their best to constantly be surrounded by opponents. The higher the number of opponents around you, the more damage the necklace will deal and the more you will heal. Most players do this by replacing their Shadow Silk Hood bind with this item; however, this makes them consume more prayer and more susceptible to gaining deaths. Because the Shadow Silk Hood decreases the number of monsters attacking you, and the Blood Necklace is most effective when as many monsters as possible attack, their pairing is not recommended. Past 100 dungeoneering, the Promethium or Primal Platebody bound in conjunction with a Blood Necklace are recommended by some over a Shadow Silk Hood. However, If a player also has a shadow silk hood bound, an activated necklace special will not cause humanoids to become aggressive to the player – the Mysterious mage sealing Yk'Lagor the Thunderous will not attack the player when hurt by the necklace's lifesteal either. This can be beneficial for those with lower combat stats who wish to dispatch enemies without the risk of entering combat.

The necklace is most respectable not for its pulse effect, but for its high offensive bonuses. While primal gauntlets have more strength bonus and the gold precision bracelet offer more accuracy, the necklace's combination of these stats makes it a powerful addition to a melee weapon.
