Primal full helm is an item released with the Dungeoneering skill update. It requires level 99 Defence to be worn. Primal full helm is the best possible full helm to obtain in Dungeoneering.
As with all Primal equipment, this cannot be made using the Smithing skill.
The only way this item may be obtained is as a drop from the Skeletal Horde boss, however it is a very rare item to find. This is owing to the fact that the drop rate of Primal equipment is affected by the combat level of the boss, and the highest combat level of the horde is very low for that of a boss, thus resulting in a very rare chance for a Primal full helm drop.
The best way to hunt for this item is to do small floors 12-14, 5:5, Complexity 6, having higher Combat levelled players in your party will increase the strength of the Skeletal Horde, and the chance of higher tier full helms.
Hunting this item is however not recommended, the Primal full helm is an impractical bind due to platebodies/platelegs offering much higher defensive bonuses, and that the Shadow silk hood is a much more effective item for the head equipment slot due to its special effect.
Compared to a Torva full helm, the stab, slash and ranged defence bonuses of a primal full helm is 6, 10 and 3 higher respectively. However, its crush bonus is noticeably poor, only 5 higher than a Dragon full helm. Like all other equipment in Daemonheim, it has 0 summoning defence but this is not important as no creature in Daemonheim uses Summoning.

Trivia
- The horns in Dungeoneering helmets get longer as their tier goes higher. The Novite full helm has the shortest horns of all Dungeoneering helmets, while Primal full helm has the longest.
- The curved shape of the horns on this helm resemble the horns on a Farseer helm.
- A Primal full helm appears in the Dungeoneering Skillcape emote, along with full Primal armour (excluding a kiteshield) and a Primal longsword.
