Pirate hat and eyepatches is the result of having Patchy sew a left eyepatch, a right eyepatch, and a pirate hat together. To get to the island Patchy is on you must do the quest Cabin Fever. Left eyepatches are lootable items from Pirate implings, which can be caught after the Rocking Out quest. Right eyepatches can be obtained from killing low-level pirates, or bought at most charter ship stores. Pirate hats can be obtained from a level 3 Treasure trail.

Trivia
- When High Detail was in beta, players wearing the hat and eyepatches experienced a glitch on their faces. Only the hat and the eyepatches would be visible, and the majority of the face would be invisible. This bug was universal to the pirate hat.
- With the Price Checker window open the Pirate hat and eyepatches can be placed inside as if they were tradeable although they are not, and the price shown is roughly 200gp higher than the Pirate's hat.
