Pirate hat and eyepatch is the result of having Patchy sew a left eyepatch and a pirate hat together. To get to the island Patchy is on you must do the quest Cabin Fever. Left eyepatches are lootable items from Pirate implings, which can be caught after the Rocking Out quest. Pirate hats can be obtained from a level 3 Treasure trail.
